Engaging community partners increases referrals, care coordination, and the individual’s options and experiences for community inclusion. Use Form 6 (community engagement) on the joint RFA to document your efforts to involve community members and ensure your organization is thoughtful about the care coordination needed to support qualified residents. Qualified residents may need case management, behavioral health support, or several additional support services to ensure quality of life and health equity. In accepting the funding from this opportunity, applicants commit to serving the prioritized population as they would serve any private pay or other resident in their care.
